Care coordination is a fundamental piece of many current healthcare delivery models, gaining popularity in cancer treatment through the role of the oncology nurse navigator. While care coordination and navigation have similarities and differences, the collection of data is critical to both. In this session, you’ll identify measures that can demonstrate navigation and care coordination productivity and quality indicators to improve patient outcomes and help to control cost.

Objectives

  1. Identify similarities and differences between the role of a care coordinator and that of a nurse navigator.
  2. Identify program success metrics for patient experience, clinical outcomes, and business performance
  3. Provide examples of data collecting tools.
