Care Coordination Throughout the Cancer Trajectory: Improving Communication, Developing Metrics, and Measuring Impact

The environment of oncology care coordination has evolved to include nurse navigators, lay navigators, and nurses who play an important role in care coordination but lack the formal title of navigator. By reviewing examples of national initiatives, you’ll discover how you can improve communication among interdisciplinary teams, develop metrics to standardize the quality of transition processes, and measure the economic and health-related impact of care coordination.

Objectives 

  1. Describe the environment of cancer care coordination and understand the unique role of the nurse navigator, lay navigator, and others who carry out coordination informally.
  2. Describe the ONS Oncology Nurse Navigator Core Competencies in the context of the interdisciplinary team involved in care coordination.
  3. Discuss issues, such as anxiety and psychosocial factors, that can pose challenges to care coordination and propose methods to meet those challenges.
  4. Identify metrics to evaluate the effectiveness and impact of care coordination on both individuals and systems.
